Here are some resources including our referral form, information leaflets about working with an educational psychologist, and some of the blank templates we use and recommend when gathering and organising information about children and young people.

 

Each time the referral form is used please also use the appropriate Information Leaflet and the Privacy Notice.

Where does the time go?
We are sometimes asked for information about the different ways that EP time can be used, with estimates of time required to carry out different pieces of work. This can be helpful if you're not sure what to commission from us.

Referral to educational psychologist - children and young people
Use this form to gather some basic background information and signed consent if it has been agreed that an assessment by the psychologist or a detailed discussion of an individual is needed.

Working with an educational psychologist

Here are four different versions of our information leaflet about meeting and working with an educational psychologist. They are differentiated into broad age bands and there is also one for parents and carers.

 

We hope the leaflets are useful as brief sources of information about the role of the EP and that they will answer some of the main questions that children and young people and their parents might have in advance of meeting an EP.

 

We're very aware that even the simplest language on the primary aged leaflet will be too difficult for some children to read. In those circumstances we hope that an adult will share the reading and in all cases where children are accessing these leaflets we hope that adults will attempt to answer as many of their questions as they are able.

 

If you are using the 'Referral to educational psychologist' form please also use the appropriate information leaflets with the form, as part of reasonable efforts to ensure that the child or young person and their parents are in a position to give informed consent to participate in the meeting with the EP.

 

We welcome feedback about the usefulness of these resources and would be pleased to answer any questions directly that are not covered by their contents.

Information gathering

Pupil Progress Tracking questionnaire
This questionnaire is used to gather information from staff and family members about a child or young person's strengths and difficulties. We will often ask for this to be completed at the start of our involvement.
